It’s important to realize, however, that problems with fertility can originate in the male or female, and possibly, both. When couples are unsuccessful at pregnancy over a period of 12 months with unprotected intercourse, fertility evaluation is warranted. Fertility evaluation should involve both male and female factor evaluation. Surprisingly, in 30% of infertile couples, the man is the sole cause, and in another 20%, both the male and female have problems. In the woman, there are lots of gynecologic, anatomic and hormonal considerations that have to be evaluated. In the male, a properly performed semen analysis is the first step.

If the semen analysis is normal, the man is generally cleared. When the semen is abnormal, the specific issues will determine the following testing. It’s essential to obtain a thorough medical history. Certain medical and environmental factors can greatly affect sperm production. We look for specific factors related to scrotal surgery, testicle injury, genital infection, radiation exposure, toxic chemical exposure, hot tub or sauna exposure, bathing habits, steroid exposure, and drug use, which are critical elements.
